On track for 32 Level III NCQA Patient-Centered Medical Home Accreditations

The hard work of our team and an amazing client on the west coast is paying off. They have successfully accredited 22 of a total planned 32 facilities for Level III NCQA Patient-Centered Medical Home. This is a mixture of family practice clinics, pediatric clinics, and primary care clinics. The remaining 10 clinics await final assessments.

NCQA ’s Patient-Centered Medical Home (PCMH) is an innovative program for improving primary care. In a set of standards that describe clear and specific criteria, the program gives practices information about organizing care around patients, working in teams and coordinating and tracking care over time.

As part of the effort to receive accreditation for Patient-Centered Medical Home, one of the things NOVACES does is help refine, and sometimes reinvent, processes for patient care to meet the criteria. It’s an opportunity to make some very dramatic improvements in each clinic’s system for coordinating patient care.
